Resource: Living will & medical power of attorney documents

It has been my intention for many years to prepare my Living Will and Medical Power of Attorney documents. I knew the importance of these documents, but I didn't have a clear understanding of how to get them done. I felt overwhelmed by the task and didn't know where to start.
So I was thrilled when I learned that it's actually much easier than I had imaged, and today I want to share the resources that helped me finally get this done!
It turns out that in Colorado all you need for your Medical Power of Attorney and to create your Living Will is to complete two documents that I have attached below. These two forms will hold up as a legal documents once they are completed, signed, and witnessed.
The forms themselves are pretty straight forward, though answering the questions on them can be a little more difficult. You will need to think through what your wishes are should you ever end up in a situation where you can't speak or care for yourself.
Use this document to appoint an agent who will communicate your healthcare wishes should you ever be unable to express them yourself. This document needs to be signed by your agent(s) and witnessed by two individuals.
This document is for you to express your wishes regarding life support should you have a terminal condition or end up in a persistent vegetative state. It also needs to be witnessed by two individuals.
While it can be difficult to discuss death with the people you love, it is important to consult with them about these questions and to communicate your wishes to them. Once the forms are completed and witnessed, copies should be shared with family members and the agent(s) appointed in them.
